It is the only track on that album where he uses his 'real' voice. However, 'Dream On' was one of the first songs he wrote (I believe he was 18.before Aerosmith) and he decided to sing it in his real voice. Then he would have breakfast and move on to other things. On Abbey Road, McCartney would come in the morning, before eating or even drinking a glass of water or whatever, to have his throat as raw as possible, and he would do one, and only one take of Oh! Darling. the man sounds like he's dying, it's just such a striking performance. He did one take (which is what's on the album) and then attempted a second take, but they had to abandon it mid-way, because by then his voice had gone. But they had to keep Twist and Shout for the end of the session, because they knew it would completely wreck his vocal chords. It actually makes his performance that much more compelling. That's actually how John Lennon got through recording most of the tracks for Please Please Me (The Beatles' debut album) which, apart from the existing singles and b-sides that had already been out for a bit, were all recorded in a single day.

Once I did, I had no idea that Steven Tyler could sing in that voice, because before that, all I heard was "I Don't Want to Miss a Thing", in which he sounds completely different.
